Because everyone’s journey is different, our helpful clinical team will assist you in the next steps to begin your child’s learning adventure! Meet Our Team

After receiving your completed intake form, our team will get in touch with you to get a better understanding of your family needs and answer any questions you have about our services at Motiv8 Children's Therapy.

*With in a week of receiving completed intake form

Pre-Authorization

After determining which services are best for your family, we will get in touch with your insurance provider for pre-authorization approval for services.

*May take up to two weeks or more after request has been submitted to your insurance.

Assessment

Our team will evaluate your families needs and create an individualized treatment plan focusing on the goals we establish.

*Within two weeks after receiving preauthorization back from your insurance.

Authorization

The treatment plan that we've created together will be submitted to your insurance for a final approval and authorization to begin services.

*Within four to six weeks after our assessment has been submitted to your insurance.
